The formula
Group and concurrent share (%) = (group + concurrent minutes) ÷ total therapy minutes × 100
Maximum group and concurrent minutes = individual minutes ÷ 3
Under US Medicare Part A in skilled nursing facilities (PDPM), group and concurrent therapy combined are limited to 25% per discipline. The calculator checks one resident, measured across the whole Part A stay. Going over brings a non-fatal warning on the MDS Final Validation Report, and there is no payment penalty. The limit does not cap how much group work a therapist may do.
This is general information, not billing, coding or legal advice. The payer's rules and your employer's policy decide.
Worked example
Example: a resident has 300 individual, 60 group and 40 concurrent minutes. That is 100 group and concurrent minutes out of 400 in total, exactly 25%. One more 30 minute group session would take them to 130 ÷ 430 = 30.2%, over the limit.
Common questions
Is the 25% per therapist or per resident?
Under US Medicare PDPM it is per resident, per discipline, across the whole Part A stay. An interrupted stay counts as one stay.
How are the minutes recorded?
Each resident's MDS records the full session minutes, coded by mode, with no splitting. Concurrent therapy is 2 residents at once, both in the line of sight of the treating therapist or assistant. A 30 minute concurrent session records 30 minutes for each of the two residents.
Does group therapy help my productivity?
It depends on how your employer credits the minutes. Crediting resident-minutes counts a group session once for each resident; crediting session time counts it once, and setup time is not counted.
