The formula
Productivity % = treatment minutes ÷ paid minutes × 100
Paid minutes is the shift with the unpaid break taken out. Documentation, team meetings and setting up a group usually count as paid but not productive time, so they lower the percentage.
Worked example
An OT in a skilled nursing facility works an 8-hour shift with a 30-minute unpaid lunch: 450 paid minutes. They have treated for 340 minutes, which is 75.6%. An 85% target needs 383 minutes, so 43 more minutes of treatment would close the gap, about one more session.
Common questions
How do group sessions count toward my productivity?
That depends on your employer. Some credit a group session once, at its length. Others credit it for each patient in the group. Ask which method your productivity report uses.
Is there a limit on group therapy in skilled nursing?
PDPM is the US Medicare payment model for skilled nursing stays. Under it, the limit for group and concurrent therapy together is 25% of a resident’s therapy minutes in each discipline. Going over brings a non-fatal warning on the MDS Final Validation Report, with no payment penalty. The concurrent and group therapy calculator checks it.
Does the KX threshold apply to OT?
Yes, in US Medicare Part B outpatient care. Occupational therapy has its own threshold, separate from PT and speech-language pathology: $2,480 for 2026.
